with Policies
Is submitted when you first join and each year upon renewal.
This is WCTV’s way of ensuring that a member understands and complies with
our Policies. When new policies are adopted, members will receive a notice.
Members are required to have a current form on file to retain active membership
and borrow equipment.
Talent
Release Form
Every person
is entitled to his or her privacy. A producer is entitled to his or her
rights of free speech and _expression. If however, before videotaping, a
person tells you that you may not use them in your video, you cannot do
so. If you do so without that person’s permission, this person may have
the right to bring a lawsuit against you as the producer. Some courts have
found in favor of persons videotaped “against their will.” A talent release
form is a method of obtaining permission to use the person’s “likeness”
in your video and may protect you against potential legal action. Also,
if the person you are taping is under the age of 18, you are required to
obtain their parents’ or guardians’ signature. Persons that you videotape
are required to sign this form. When you submit your videotape to us with
our required signed Talent Release form, you are confirming that you obtained
the required permissions from persons who appear in your program prior to
submitting your tape for cablecast.
Location Release Form
For events including
large groups of people, such as concerts, meetings, etc., it may be difficult
to obtain individual “Talent Release” forms. A “Location Release Form” signed
by the person responsible for the performance or event is a method of obtaining
permission to videotape on the premises. If you are videotaping a program
and including recognizable individuals, you will need to obtain the proper
permissions from persons in your program as well as have the “Location Release
Form” signed.
Equipment Checkout Form
Required
for signing out any of WCTV’s portable equipment. A staff member will assist
you with putting together the equipment you need and signing it out. Both
the member who requests the equipment and a staff person are required to
sign this form when the equipment is taken out or returned
Cablecast Request Form
There are
two different forms used - The general “Cablecast Request Form” is used
for programs produced “in house” or at WCTV. The “Import Program Form” is
required for a program that is not produced at WCTV. (Programs submitted
without this signed paperwork will not be cablecast until the form is completed.)
Program Proposal Form
Before a member submits a program, the idea should be approved by the Executive
Director. This form is a way to let the WCTV know what your project is,
and how and when it is going to be produced.
Equipment Repair Form
Occasionally,
equipment may malfunction. In order to keep equipment working properly,
members are asked to fill out this form to inform the staff of equipment
problems and have equipment repaired in a timely manner.
Studio/Facilities Request Form
When you
choose to produce an “in studio” show, you are required to fill out a form
to reserve the studio prior to planning your production and a staff member
will talk to you about available time on the studio schedule. Members who
choose to produce a program in the studio are required take the Studio Production
class. We recommend a “refresher” course for members who have not used the
studio facilities for some time. Staff may assist in the production but
the producer is responsible for arranging for production crew
Log Sheet
One of the
most time-consuming parts of editing is scrolling through your tapes to
choose video. By reviewing your videotapes before you begin editing and
“logging” your footage, you will save time when producing your final tape.
We recommend using log sheets to help you locate and select footage during
your final editing session
